This plain-English overview describes how Tal Dagan MD PC may use and disclose protected health information and how patients can exercise their rights.
The practice is required to protect the privacy and security of protected health information, provide this notice, follow the notice currently in effect, and notify affected individuals when a breach may have compromised protected information.
Health information may be used or disclosed for treatment, payment and health-care operations; to comply with law; for public-health and safety activities; for oversight, workers' compensation and law-enforcement purposes when permitted; and in other situations described by law. Other uses generally require written authorization, which may be revoked in writing subject to legal limitations.
You may request access to or a copy of your record, ask for a correction, request confidential communications or certain restrictions, receive an accounting of certain disclosures, obtain a paper copy of the notice and choose someone to act for you when legally authorized.
